No? Let’s try to make some changes - going back to the basics - to see if we can check those boxes! 

Also, nearly all nursing couplets will eventually evolve. Newly born, 1 month, 4 months, 6 months, 10 months, 18 months... your nursing positions will change and adapt. There are good reasons I might recommend to avoid a certain position in the early days (traditional sitting up cradle, for example, doesn’t give baby good positional stability/front body contact and gravity works against you) but generally as babies get bigger and better there’s a lot more flexibility.

So obviously there’s no meter or gauge on the breast to tell you exactly how much baby took at a given feeding. I know that sometimes that can cause anxiety in new parents or in experienced parents that may not have breastfed with previous children or pumped and bottle fed! I get it. You just want to know you’re baby is getting enough milk to thrive! 

One of the best signs is weight gain, but home scales aren’t usually accurate enough for tracking that and after the first 2 weeks pediatrician appointments can be more than a month apart. So what can you do? 

Watch for these positive signs - 
✅ Frequent, active feedings - 8-12+ times per day in first few months
✅ At least 5-6 soaking wet diapers 
✅ Yellow, seedy stools (2-3 per day in first few weeks but older babies may go a day or more between poopsplosions) 
✅ Swallowing you can hear every 1-2 sucks 
✅ Periods of content, happy baby 
✅ Pain free latching

If you can check all those boxes, that’s a good sign things are going well! If you still have concerns or you’re not checking those boxes, check in with your IBCLC or pediatrician for a weight check and feeding evaluation.

Let’s talk engorgement! Ouch!

Let’s talk engorgement! Ouch! 

Engorgement is like the extreme version of milk full breasts. The most common time for this to occur is within the first 48 hours after the mature milk surges (“comes in”) around day 3-5 postpartum. But it could also happen at other times, for example, when there are changes in feeding frequency, a separation of the nursing couplet, mom out of town for work, baby weaning… 

It’s important to understand that engorgement IS milk filling the breast, but it’s also SWELLING! This swelling can make the skin taught and shiny, it can puff up the areola and flatten the nipple. In addition to being incredibly uncomfortable, it makes it more difficult for baby to latch - which means less milk removal - which means more engorgement. ?

So what do you do about it?? You need to treat BOTH symptoms - the milk fullness and the swelling. 

Fullness is treated by frequent effective removal of the milk. Including hand expression for comfort. (Check out yesterday’s posts!!)

Swelling is treated with gentle massage (video and step by step instructions to come), cold compresses after nursing/pumping, reverse pressure softening (photos and video to come), clean cabbage leaves (science on this is still inconclusive though anecdotally many report positive results), and NSAIDS assuming you have no allergies or contraindications.
